The phase III ENDURANCE trial showed that two years of lenalidomide maintenance therapy offers the same long-term survival benefit as indefinite treatment for patients with standard-risk multiple myeloma who are not transplant candidates. After nearly seven years, survival rates were nearly identical, but longer treatment caused more side effects and a slightly higher risk of secondary cancers. This finding supports a fixed-duration treatment approach, improving quality of life by reducing side effects and treatment burden without compromising survival. The study was led by the ECOG-ACRIN Cancer Research Group and co-authored by leaders from the International Myeloma Foundation.
