A new prospective multicenter study published in Dermatology and Therapy finds that Castle Biosciences' DecisionDx-Melanoma i31-SLNB test more accurately identifies patients at low and high risk of sentinel lymph node (SLN) positivity in cutaneous melanoma than the Melanoma Institute Australia (MIA) nomogram. The i31-SLNB test showed a 2.6% SLN positivity rate for low-risk patients, below the 5% threshold for avoiding biopsy, compared to 5.8% for the MIA nomogram, which failed to identify patients below this threshold. This improved accuracy supports better clinical decision-making on whether to perform sentinel lymph node biopsy, potentially reducing unnecessary surgeries and complications. The study reinforces the value of integrating tumor biology with clinical factors for personalized melanoma care.
